Community Connection of Minnesota, Inc. was started in 2000. The company provides adult foster care in a group home setting to individuals with traumatic brain injury (TBI) and related conditions. Each home has up to four adult residents. The company has eight homes in the Duluth area and is growing as the need for TBI services continues to increase in the state of Minnesota. In October 2005, John Mack, one of the owners of Community Connection of Minnesota, Inc., died unexpectedly from a brief illness. John loved working with our residents and they loved him. We honor his spirit and love for his work through a firm commitment to continue the services of Community Connection of Minnesota, Inc.

SCOPE OF SERVICES
We are always interested in meeting with new individuals who may benefit from our services. We recognize that each person has unique needs and abilities. Community Connection is committed to developing individualized care plans with the assistance of the resident, the resident’s family, medical staff, and case manager. Our “Personal Futures Planning ” program with residents includes:
- Welcome -Let’s Get Started – This is an introductory process designed to help the residents identify goals that they would like to work on. These goals may include attending day programming, school, employment, and independent living.
The resident, along with the multi-disciplinary team of health care professionals, case managers, family, legal guardian, and Community Connection of Minnesota, Inc. staff, develop short-term and long-term goals for the Individual Service Plan. If specific goals are already established, they will be incorporated into the care of the resident.
This is the time the resident spends working at adjusting to the community and their surroundings. By focusing on cognitive as well as behavioral issues, each resident will come to understand the importance of house rules, medication and appointment compliance as well as social and independent living skills. - Where Are You Now - In this stage of the program a plan is developed to assist the resident in reaching their ultimate independence level. This process helps to identify barriers that may interfere with the resident reaching their goals. This process is also helpful in identifying the strengths of the resident. Each resident continues to use skills learned in the Let’s Get Started phase as well as progresses to the long-term goals established by the multi-disciplinary team. Residents also concentrate on developing skills that allow them to cope while in the community. At this time the resident works on the least restrictive living skills. Our program works at developing individual goals and desires to match abilities with basic living skills. The plan is developed with the individual needs of the resident in mind. Community Connection offers support in a variety of areas including community integration, spirituality, culture and diversity, medication monitoring, daily living skills, money management, behavior management, physical fitness, and nutrition.
Where Are You Now also works at helping residents establish healthier relationships with family and friends. Productive home visits are encouraged and residents work at preparing for a successful visit. - Getting on Your Own - Once progress has been made toward the achievement of individual goals, the resident and Community Connection staff can begin to explore a more independent living environment for that person. A multi-disciplinary team will work with the plan for all individuals who move to Getting on Your Own, whether they have been residents with Community Connection of Minnesota, Inc. or whether they have come to us for these services. Once an individual moves to the new residence, an ongoing assessment for success is initiated.
OTHER SERVICES
All-Resident Meetings are held on a monthly basis to provide the opportunity for residents to meet and interact with members of the Community Connection care and administrative teams. Residents are encouraged to provide feedback about the services they are receiving.
Activities are an integral part of the residents’ lives at Community Connection of Minnesota, Inc. Residents define the types of activities with which they would like to participate. Coordination of Care is provided for all residents with their medical care providers in the local or referral communities. Each resident is able to receive the appropriate care from physicians, consultants, or allied health services as is needed . Community Connection of Minnesota, Inc. will assist the resident in meeting these needs by providing transportation, support services at the visits as needed, and follow-through per the professional recommendations.
Staff Education and Training is ongoing to support the high quality of services and interactions with residents of Community Connection of Minnesota, Inc.
Quality Management Program reflects our commitment to quality assessment of our resident services, as well as all company services, on an ongoing basis. Participation in the quality review process includes input from residents, employees, and other individuals affiliated with Community Connection of Minnesota, Inc.
